Congratulations on being selected as a recipient of a scholarship from the Sioux Falls Area Community Foundation! We believe that students like you are the thought-leaders and change-makers of tomorrow, and we are proud to support you on your educational journey.

To formally accept your award(s), complete the form below. Your answers will be used to help the Community Foundation process your award check and also shared in our official announcements of this year's scholarship recipients.
 

Please note: If you have received multiple scholarships from the Community Foundation, you only need to submit this form once.

Confirm Your Educational Information

The scholarship award check will be sent directly to this institution in June. If you decide to attend an alternate educational institution before the Fall semester, please inform the Sioux Falls Area Community Foundation.

NEXT STEPS

- The Community Foundation will receive your acceptance form once you hit the Submit button below.
   You will not receive a confirmation email upon submission.

- Your scholarship check(s) will be printed in June and sent directly to your educational institution.   
   You will receive an email notification once your scholarship check has been printed and mailed.

- Your educational institution will process the scholarship check and apply it to your billing statement.
   If you do not see this scholarship on your billing statement, please contact your school's Financial Aid Office
   or Business Office to inquire about the processing of these funds.
